Please Note: The Business Name you enter below should be the legal name of your company. This name is also used for Card Association registration purposes, and will be presented as your business descriptor on your online customer’s monthly credit card statements. This name should be consistent with all processing requests.
IMPORTANT: Prior to activation you will be required to provide proof of your business and personal identification.
Additionally, you will be required to provide a form of identification for both the Business Name and the Principal Name that you list on this application. Please ensure that the Business Name and Principal Name that you enter in the fields below match the identification EXACTLY. Failure to do so may result in the rejection of your application.

As a part of our account review process we require the contact information of the principal or primary business decision maker. This information will be used for all primary communications regarding your account and will remain on file in our system.
